Eligibility

Families with children, teens, or young adults through age 21; youth up to age 26 with foster care involvement; individuals enrolled in CCS/CCS Whole Child Model; children and families at risk of or experiencing homelessness; and children with mental health or substance use conditions. Caregivers may also receive support. Services are for individuals with Managed Medi-Cal

Application process

Referrals are primarily made through Medi-Cal managed care plans. Community partners and service providers may also inquire about referrals by email.

Serve's Medi-Cal recipients who are experiencing mental health challenges often in addition to homelessness, substance use issues or histories of trauma and abuse. Services include therapy, case management, living skills support, medication management, participant groups and peer mentoring.  CCTAY works in close collaboration with Contra Costa County Behavioral Health Services.
