Case/Care Management | CARE Center - Trinity Center - Walnut Creek
Provides a respite and services for low-income adults and/or homeless in the Walnut Creek area. Provides job and housing search assistance, mailbox, showers, clothing, a washer and dryer, case management, alcohol and other drug services, advocacy and assistance in accessing county, state and federal services, including SSI and Food Stamp benefits. Client in need of housing navigation ONLY, must contact other coordinated entry providers. Specializing in single adult men and women; very limited services for families; no childcare is provided. Winter shelter at the Armory is available to current clients of the center. Clients may become members by walking in to center.

Application process
Walk-in to register as a new member, complete first arrival questions and agreements. Full intake will be scheduled with a member advocate. The intake will be done by phone and scheduled in after the first arrival questionnaire.
