Eligibility

Family income is no more than 200% of federal poverty level; child is not enrolled in Medi-Cal or other vision insurance, child is 19 years old or younger, valid social security number, child has not used the program during the past 12 months.

Description

Distributes Eyes of Hope gift certificates to low-income children through VSP's network of community partners. Parents and guardians interested in seeking assistance through the Eyes of Hope program must work through the VSP Provider. VSP does not issue gift certificates directly to the public. The local providers are listed on the website. School nurse and some youth programs may have applications and be able to provide assistance.
