Eligibility

Parents or caregivers with children age 0-3 who reside in West Contra Costa County. After evaluation child must have a 50% delay in one area of development or 33% delay in two areas of development.

Application process

Call extension 29530 for English or 29526 for Spanish Intake line to request a registration packet and appointment. Once packet is received case manager will complete an evaluation of parents concerns and child's needs.

LIFT (Learning for Infants and Families Together) team for infants to 3. Provides developmental playgroup which includes facilitated and speech play, structured circle-time, snack-time, outside time, 1-2 home visits per month and case management as needed.
