Eligibility

Age 3-20, psychiatric hospitalization in past 24 months or current behaviors put participant at risk for hospitalization, or participant previously received therapeutic behavioral services. Must be referred by primary mental health provider and approved by Contra Costa County Behavioral Health.

Service area

Contra Costa County
Offers one-to-one behavioral intervention mental health program. Works in collaboration with children, youth, parents, caregivers, foster parents, group home staff and school personnel to reduce and manage challenging behaviors, as well as strategies and skills to help child and youth in their current environment. Provides children and youth transition assistance to lower levels of care.

Community-based mental health service program for young adults (16-26) living in Western to Central Contra Costa County. Serve's Medi-Cal recipients who are experiencing mental health challenges often in addition to homelessness, substance use issues or histories of trauma and abuse. Services include therapy, case management, living skills support, medication management, participant groups and peer mentoring.  CCTAY works in close collaboration with Contra Costa County Behavioral Health Services.
