Case/Care Management | Black Infant Health Program (BIH) - FMCH CC Health
Uses a group-based approach with client-centered services to help women develop life skills, learn strategies for reducing stress, and build social support. Participants participate in weekly group sessions, supported by one-to-one services, designed to help client access self strengths and set health-promoting goals for client and the baby. Focuses exclusively on empowering Black and African American women by connecting them with the vital care and support needed to promote healthy behaviors during pregnancy and continuing after the baby is born.
