Services provided by phone and telehealth. Provides a family-driven, team-based program providing advocacy, peer support, and coordinated care for families facing challenges and life stressors.

Provides high fidelity wrap around model assistance that helps the family and child coordinate services and supports in a way that empowers them to meet their needs as they define them. Partners with families to develop a family-driven plan that promotes and helps with self-advocacy, system navigation, referrals for housing and employment as well as substance abuse issues.

Provides integrated behavioral support services for students in K-12 in the Pittsburg Unified School District and in 3 schools in Antioch Unified School District (Antioch Middle, Dallas Ranch Middle and Park Middle School). Develops tools and learning environments for academic success across all levels from teacher consultation, student counseling and whole school support. Provides comprehensive support including, in-class/playground support (elementary level), individual, group and family therapy, teacher consultation, crisis intervention and community outings.

Offers one-stop center for people affected by domestic violence, sexual assault, elder abuse, child abuse and human trafficking. Center helps people in crisis to find hope, safety and healing by being a one-stop center where many service providers are readily available.

Offers one-to-one behavioral intervention mental health program. Works in collaboration with children, youth, parents, caregivers, foster parents, group home staff and school personnel to reduce and manage challenging behaviors, as well as strategies and skills to help child and youth in their current environment. Provides children and youth transition assistance to lower levels of care.

Offers one-stop center for people affected by domestic violence, sexual assault, elder abuse, child abuse and human trafficking. Center helps people in crisis to find hope, safety, and healing by being a one-stop center where many service providers are readily available including connections to telehealth appointments, mental health counseling and more.

Provides wraparound services which include planning process and a comprehensive range of supports and services to children and families who have complex needs. Services can be offered in home and/or community settings, and can include the following: team meetings, creating action plans, case management/linkage, crisis intervention, collateral services, plan development, flex funds, peer support, mental health assessments, and evaluations. Serves Families in East Contra Costa County.

Offers a wraparound program through MHSA, serving adults aged 25+, with serious mental illness who are homeless or at-risk of homelessness. The program provides high level case management through the recovery model including support services such as housing, vocational rehabilitation, and counseling.
