Community-based mental health service program for young adults (16-26) living in Western to Central Contra Costa County. Serve's Medi-Cal recipients who are experiencing mental health challenges often in addition to homelessness, substance use issues or histories of trauma and abuse. Services include therapy, case management, living skills support, medication management, participant groups and peer mentoring.  CCTAY works in close collaboration with Contra Costa County Behavioral Health Services.  

Provides bilingual services that support mental health, developmental disabilities, substance use recovery, housing, employment, and reentry programs. Additionally they assist individuals and families with counseling, job training, peer support, and community engagement.

Provides a therapist referral service of gay, lesbian, transgender and bisexual therapists both online and by phone, offers seminars, consultations, and speakers by members on such LBGTQ-related topics as homophobia, coming out, gay marriage, HIV/AIDS, alternative families, parenting, gender identity, S/M/B/D, as well as how anxiety and depression affect our community.

Provides suicide prevention mobile application called MY3. Developed by the Know the Signs Campaign. Connects clients with their three primary contacts when having thoughts of suicide. Offers customizable safety plan and resources page. Cellular phone application is available for clients with iPhones or Androids. Website has extensive information.

Provides mental health/wraparound/psychiatry services to children and youth ages 5 through 18 who are West Contra Costa residents, including early intervention for children, birth to 5.

Supports families of individuals with mental illnesses, educates families and the public about mental illness, advocates for improved treatment and treatment facilities, fight stigma and discrimination, supports legislation that helps those with neurobiological brain disorders, improves private and public sector policies, increases outreach to people with mental illnesses and their families. Refers clients to local support groups provided through other organizations or local churches.

Provides online listing of licensed Marriage and Family Therapists serving the East Bay giving contact information. Information can be selected by language, gender, specialty, insurance plan, therapy type. The therapists description may or may not include information on sliding scale fee for service; individual should call the therapist for this information.

Works with county mental health as a bridge between the system and explaining to families who are dealing with a family member 18+ and dealing with behavioral health issues. Services include assisting families in navigating the mental health system, answers questions regarding SSI and SSDI problems, benefits questions, rehabilitation programs, day programs, county housing resources, out of the county placements, and medication issues. Therapy and psychiatrist referrals in both the private and public systems. Meets with families within the county mental health system of care.

Provides an overview of mental health services offered by County Mental Health and nonprofits. Course is a partnership with Behavioral Health Administration, the Family Justice Center, and NAMI Contra Costa.

Clinical High Risk (CHR) program works with young people at risk for or experiencing very early symptoms of psychosis, prior to a first psychotic episode. First Episode Psychosis (FEP) program works with those who have had a first psychotic episode within the past year. Services are designed to support recovery and functioning and to prevent the occurrence of an initial or subsequent episode of psychosis. Offers individual, family and group therapy, care coordination, multifamily groups, employment and education support, occupational therapy, peer support, substance use counseling and psychiatric services.
