Provides Screening, Diagnostic and Treatment (EPSDT) counseling for children, adolescents, families and groups age 5 to 21 in home, school or office. EPSDT process is in place to assure that medically necessary health care is received in order to correct or ameliorate a defect, physical or mental illness, or a health condition identified through a screening.

Provides limited intake for Christian based full-time year long program. Services include, meals, case management, spiritual counseling.

Provides general information on developmental disabilities, systemic advocacy for people with mental retardation, cerebral palsy, autism, epilepsy, and conditions similar to mental retardation. Also conducts Quality Assessments for clients of the East Bay Regional Center, supports People First organizations and provides public education and information. Assists local councils on developmental disabilities in planning, coordinating, monitoring, advocacy, and evaluating services for individuals with developmental disabilities and their families to enhance independence, productivity and inclusion.

Provides a general acute care hospital with services open to members, emergency services open to anyone regardless of ability to pay. Offers health education classes, some of which are open to the public.

Provides temporary safe homes where parents may voluntarily have their children cared for during a family crisis. Crisis situations may include homelessness, unemployment, incarceration, drug addiction, medical illness, hospitalization, or any other crisis circumstance that makes it difficult for the parents to care for their children. The staff offers support by helping new and previously helped clients find needed resources for their families. Safe Refuge for Children + Families is a faith-based organization that recruits its volunteers through churches. These local churches support the families hosting the children of the families in crisis. The Safe Refuge volunteer host families are screened (background checks, home studies, training, etc.) and approved to be safe. This is a voluntary program and the families in crisis always retain all of their parental rights and custody.

Serves as a cooling center during summer heat wave. Clients must call to confirm open hours and location.

Provides 15 to 20 pounds of fresh produce two times per month to qualifying households that are in need of food assistance. Produce is distributed from the Food Bank of Contra Costa and Solano Mobile Trucks.

Provides a respite and services for low-income adults and/or homeless in the Walnut Creek area. Provides job and housing search assistance, mailbox, showers, clothing, a washer and dryer, case management, alcohol and other drug services, advocacy and assistance in accessing county, state and federal services, including SSI and Food Stamp benefits. Client in need of housing navigation ONLY, must contact other coordinated entry providers. Specializing in adult men and women limited services for families; no childcare is provided.

Provides a one-stop family resource center serving families of children with disabilities and special needs. Care Parent Network helps families meet the challenges of parenting a child with special needs. Care Parent Network offers one-to-one support, information, virtual workshops, referrals, mentors parents, and support group meetings. Provides information about transitioning a child with intellectual and developmental disabilities from the public-school system into programs for adults. Offers two father support groups available in English and in Spanish as well as an African American support group. All services are in English and Spanish. Offers virtual support group for black/multi-racial families who are raising individuals with special needs.

Partners with Resources for Community Development and provides on-site services at three Affordable Housing apartment complexes: Lakeside Apartments (Concord), Los Medanos Village (Pittsburg), and Bella Monte Apartments (Bay Point) accept housing applications at each of those sites. After residents are housed, Hope Solutions provides support services to help formerly homeless residents maintain stable housing.

Provides advocacy, client program planning, coordination of generic services, and consultation. Provides case management services through its own units and through two other contracted delegate agencies, La Familia Counseling Services and Asian Community Mental Health Services. An active client of the Regional Center must participate, at a minimum, in planning meetings to develop or revise an Individual Program Plan (IPP), or an Individual Family Service Plan (IFSP) for a child under 3 years of age. An IPP identifies services and plans necessary for a client to achieve agreed upon objectives.

Supports families of individuals with mental illnesses, educates families and the public about mental illness, advocates for improved treatment and treatment facilities, fight stigma and discrimination, supports legislation that helps those with neurobiological brain disorders, improves private and public sector policies, increases outreach to people with mental illnesses and their families. Refers clients to local support groups provided through other organizations or local churches.

Provides counseling and police escorts to sexually abused victims through: 24-hour crisis line counseling, in-person support, including accompaniment to hospitals, police interviews, and legal proceedings.

Provides BACN Boutique which is a fully stocked, free boutique filled with baby diapers, formula, children’s furniture, toys, books, clothes, and more for families in need. Site also offers a food pantry available 24 hours daily.
