Provides job training and placement for individuals with developmental and physical disabilities. The goal of our program is to introduce consumers into competitive employment. The program provides a full range of services which enable people with disabilities to access and succeed in competitive employment. The program goal is to provide full access to employment through the provision of individual support services for people who have historically been excluded from employment. The services have a four step process: assessment, job development, pre-vocation, and job coaching.

Provides a variety of medical services including family medicine, women's health, pediatrics, chronic disease management, immunizations, health education, HIV testing, optometry services and dental care. Optometry services also available, hours have been expanded; please call for hours and appointments.

Provides support to vulnerable populations with wrap around support, housing navigation, housing deposit assistance, housing stabilizing, and retention services.

Offers a variety of exercise/fitness classes for adults. Classes include, yoga, tai chi, zumba, pilates, weight training, including meditation. Classes may fill up fast or may be cancelled depending on registration.

Provides emergency shelter and interim, non-congregate housing for single adults. In addition to providing basic needs of shelter, shower, food, and clothing, the program includes care coordination and housing location services assisting residents with move-in and short term rental assistance where appropriate. In addition to housing, employment, and clinical support services, the program offers on-site health care services provided by the county and linkages to legal aid and other social and human resource services.

Provides services to young children, ages 0-5, who are deaf, hard of hearing or who have severe speech and language delays. Offers comprehensive programs in early education in combination with extensive therapy (language, speech, auditory, occupational, and physical therapies), home visits, and family education and support. Provides parent and child playgroups (12-36 months) which provide social interaction and organized educational activities designed to promote spoken language and sign language communication skills.

Acts as the liaison between a pregnant woman and the services needed to provide the child a healthy start and the mother a healthy pregnancy. Coordinates initial prenatal appointment and referrals to programs for pregnant women i.e. WIC. Makes referrals as needed, offers genetic counseling, prenatal testing, brief counseling on substance abuse issues and community outreach.

Accepts and redistributes clothing items to anyone in need. Items will be given out as they become available.

Offers a centralized system that provides community resource navigation and referral for children with potential developmental, learning or behavioral concerns. Offers early identification and detection of at-risk children and helps families find resources to support their needs and development. Provides information regarding child growth and learning, as well as developmental milestones of each age.

Provides mental health services to children and youth ages 5 through 18 who are Central/South Contra Costal residents including early intervention for children and wraparound services. Special intervention for children under 5, call main number for information.

Provides general information on developmental disabilities, systemic advocacy for people with mental retardation, cerebral palsy, autism, epilepsy, and conditions similar to mental retardation. Also conducts Quality Assessments for clients of the East Bay Regional Center, supports People First organizations and provides public education and information. Assists local councils on developmental disabilities in planning, coordinating, monitoring, advocacy, and evaluating services for individuals with developmental disabilities and their families to enhance independence, productivity and inclusion.

Provides a food pantry for West Contra Costa residents in need of food.

Offers ADA regulations, technical assistance materials and publications are available in standard print as well as in alternate formats for people with disabilities. Provides ADA information line to answer questions and offer technical assistance to the public.

Provides administration over all San Pablo city parks and recreational activities, art gallery, museums and community centers. Offers summer sports camp, virtual preschool programming, and a variety of classes for children, teens, and adutls including cooking, dance, exercise, martial arts, yoga, tai chi and sports.

Administers five elementary schools and one middle school in Walnut Creek. Programs and classes are available for special needs children, ages 0-22; call for information.

Provides academics and career readiness, including job application workshops and support, financial literacy workshops, after school tutoring, California High School Exit Exam preparation and support. Offers a full computer lab on-site with staff who provide job search and resume assistance.

Provides a broad range of services to adults with psychiatric disabilities. Issues include, depression, bipolar, schizophrenia, PTSD, anxiety, relationship conflict, developmental concerns, prescription management, family communication and parenting issues. We offer two programs: Contra Costa Outpatient Pittsburg and Community Support Program (CSP). Outpatient offers services for mild and moderate psychiatric disabilities including weekly individual and group psychotherapy. CSP offers intensive services for severe psychiatric disabilities including case management, medication management, 24/7 hour crisis line, individual/group/family psychotherapy, housing/education/vocational support, peer support, and dual diagnosis treatment.

Supports child care programs by facilitating and increasing access to care. Services include assistance with finding and selecting child care and family resources, guidance and support applying for subsidized child care, technical assistance to child care professionals, child care needs assessment upon request, and parent education opportunities.

Maintains a list of Medi-Cal prenatal providers in Contra Costa County. The providers offer an Initial assessment, trimester reassessments, postpartum assessment, interventions and follow-up services in obstetrics, nutrition, health education, psychosocial services, individual case coordination, prenatal vitamin/mineral supplements, and referrals to related services. Administers eighteen Preschools, thirty-six Elementary Schools, one K-8 School, six Middle Schools, six High Schools, along with Alternative Schools, and an adult education program as well as special education programs.
