Provides a broad range of services to adults with psychiatric disabilities. Issues include, depression, bipolar, schizophrenia, PTSD, anxiety, relationship conflict, developmental concerns, prescription management, family communication and parenting issues. We offer two programs: Contra Costa Outpatient Pittsburg and Community Support Program (CSP). Outpatient offers services for mild and moderate psychiatric disabilities including weekly individual and group psychotherapy. CSP offers intensive services for severe psychiatric disabilities including case management, medication management, 24/7 hour crisis line, individual/group/family psychotherapy, housing/education/vocational support, peer support, and dual diagnosis treatment.

Offers support to youth navigating the juvenile system with diversion classes, Know Your Rights workshops and community service opportunities for court ordered requirement. May be referred through probation counselor, court or parents who need help with their adolescent. Diversion program is 8 weeks and there are three groups throughout the year.

Offers a supplemental nutrition program that helps low income pregnant women, mothers with infants, infants, and young children eat well, be active and stay healthy. Provides supplemental food benefits using a WIC card. Offers nutrition and breastfeeding support and education, and referrals to other health and community services. Provides fair housing counseling and investigations to all Contra Costa County residents (except Antioch). Also provides fair housing training and presentation to both landlords and tenants throughout the county.

Courses are both court ordered and open to non-court-ordered. Offers juvenile anger management and domestic violence classes for individuals ages 16-18. Offers family reunification educational program for families who have experienced physical, mental, emotional, or psychological abuse/trauma.

Recruits and provides foster family certification to enable families to be eligible to provide a crisis home for youth needing a safe environment for youths, ages 0 to 17, who are experiencing crisis while NCFC counselors work with the youth and family to reunite them, or develop mutually agreed upon alternatives. Also provides Long-term Foster care for children referred by Social Services in Contra Costa, Alameda, and Solano counties. To be eligible, family must possess the ability to relate effectively to and adequately supervise youth, have space to temporarily house one or more young people, possess the ability to be certified by the NCFC as a Foster Family, and pass child abuse and criminal background checks.

Offers three clinics that provide a variety of patient services, including general vision care, binocular vision, comprehensive eye exams, contact lens clinic, dry eye clinic, emergency care, infant/toddler exams, low vision exams, medical eye exams, refractive surgery center and vision function exams.

Offers a comprehensive array of early intervention services for children at risk or with intellectual and developmental disabilities. Services are provided for families and children at risk for developmental delays, or at risk due to environmental and other factors or mental health challenges. Services are available in structured center based programs as well as in home and community settings. Group and individual services are available.

Provides temporary financial assistance, counseling, supportive services, employment training, education and career services to Native American families in Contra Costa County that belong to a federally recognized tribe.

Provides a general acute care hospital and associated services. Serves as the only trauma center for Contra Costa County and portions of Solano County. Housing Support Program (HSP): HousingWORKS! provides supportive housing services to homeless CalWORKs families to help them achieve self-sufficiency and housing stability. Supportive services include assistance with housing search and temporary/permanent housing placement, time-limited subsidy assistance using Rapid Rehousing and Housing First models and on-demand in-home services. Eligible families are referred through EHSD and the H3 County Coordinated Entry System. Offers Youth and Young Adult (YYA) Housing Support program which provides time-limited subsidy assistance and in-home support services to transition age youth to help them achieve permanent housing and independence the CC County Coordinated Entry System.

Provides a variety of programs for families with young children, age 0-5. Information on HMG playgroups and various activities available on website. . Offers virtual and in-person parenting classes and workshops and online activities for families such as art, music and more. Clients can meet with a community resource specialist and socialize with other parents. Center offers Kinder Readiness classes, ages and stages questionnaire and virtual HMG playgroup.

Provides administration over all city parks and recreational activities, aquatic facilities and community centers. All parks are open from sunrise to sunset. Park restrictions are listed online including special reservations.

Public lobby serves as a place to cool during summer heat waves, call to confirm hours and space availability.

Provides early childhood education for Bay Area children ages 3-6, elementary, middle school, and high school education, as well as a career center, transition services, and resource center for Deaf for individuals from San Louis Obispo to the Oregon border. Students from nearby communities attend on a day school basis; a residential program (age 5+) is available for those at a further distance. Referral needed from student's local school district. Offers sign language classes for parents and parent workshops.

Provides a food cards for Walnut Creek or Concord residents and needs based services which may include; utility and financial assistance, and vouchers for clothing that can be used in lieu of cash at St. Vincent de Paul thrift stores for discounted clothing or household/furniture items.

Supports child care programs by facilitating and increasing access to care. Family recertifications and eligibility will continue. Provides free services related to child care to parents/guardians and child care professionals in Central Contra Costa County. Services include assistance with finding and selecting child care and family resources, guidance and support applying for subsidized child care, technical assistance to child care professionals, child care needs assessment upon request, and parent education opportunities.

Provides a variety of medical services including family medicine, women's health, pediatrics, chronic disease management, immunization and HIV testing. There are no prenatal, dental or vision services at this location.
